If the offer states that you must complete the second page - does this mean the second web page that comes up when you click on the offer or somewhere past the survey towards the end where the silver, gold, platinum offer pages come up?

It means the second page that loads after you submit your e-mail. So you fill out your info with valid name, address etc. and click submit, that's it. You're not required to go through the survey.
